Personal Emergency Response System

Astella Home Health provides a PERSPlus (Personal Emergency Response System) for our patients receiving home health care. PERSPlus is an “in-home call button” providing 24/7 connection, 2-way communication which places the patient in touch with our care team to provide them with the right care, at the right time and in the right place. Designed to be used in ALL situations NOT just for emergencies! 

• Prevents unnecessary emergency room visits,  hospitalizations, frequent physician office visits and premature institutionalization. 
• Helps support independence at home to provide social reassurance, relieve isolation and improve quality of life. 
• Members receive an initial 15-day wellness call and ongoing monthly communication  from our CST Care Center. 
• Available for patients with and without a landline phone access.
